Judo Belt Test Forms
All students must turn in a completed Judo Belt Test Application no later than 7 days before test day.
Forms must be:
Signed by a parent/guardian
Signed by one of their teachers (to confirm good academic standing)
You can drop completed forms at the front desk or email them to info@5050bjj.com.
No form = no test!
Eligibility Requirements
Students must be enrolled in the Juniors (1st grade & up) or Teens Class
Attend Tuesday or Saturday Judo at least once per week
Minimum of 30 Judo classes before their first belt test
Students must hold their belt rank for at least 9 months before testing again
Final approval is required from Coach David Terao
Testing Schedule
Judo Belt Tests are held quarterly on the last Tuesday of the month (unless otherwise noted)
Eligible students will be notified once applications are approved
