Last day to turn in Stripe Test Forms: May 26

Stripe Test Forms
Make sure to turn in your signed stripe test form before test day, we can’t accept any last-minute forms on the day of the test.

You can drop it off at the front desk or just email it to info@5050bjj.com.

No form = no test!

Attendance Requirements

• Juniors must have attended 14 classes between each stripe.

• Teens need 10 classes between stripes.

• Retesting? No need to reapply, just let the front desk know you're retesting next week so they include your name on the testing list.
